Health Records

NEW and returning students entering PK, Kindergarten, 2nd grade, and 6th grade will need to have the following health records on file in order to be compliant according to the Illinois State Board of Education:

• Birth certificate
• Complete immunization record signed by a doctor (prior to October 15th)
• Dental exam
• Vision exam

Sign Up for monthly shifts at New 2 You Quality Resale store.

Calvin families can earn tuition credit by working at least 3 hours per month. Anyone can work a shift to earn tuition credit for a particular family or for Calvin Christian in general.
